Before he made the One ring he could shapeshift to many forms such as; Werewolf Sauron, giant serpent, vampire and a nice pleasant form, it was with that form he went among the Elves and forged the Rings of power. After the sinking of Numenor which Sauron went down with he lost the ability to appear fair and pleasant, I still think he could change though, but not into anything nice. It was after the destruction of Numenor he had his worst most horrible form, black and horrific. Some people think he had 1 single eye but not sure about that one.
At the time of War of the Ring he had a shape like a man but greater not gigantic, he was terrible to behold, none could match him!

I'm not sure if you could count it as a description, but in the Silmarillion Sauron is mentioned to origionaly have been a Maia of Aule and to have been as fair and bright as the spirit that carried the sun across the sky. (sorry i forget her name.)So maybe that was his true form before he became such a naughty boy.

Not exactly. He lost the ability to shift to any form that appears fair to men - i.e. he could not cunning deceive others with innocent disguises and the like (as he did in Numenor).
