Well, this isn't known for sure. It might be Eorl (indicated by the lotr Appendices' "(Eorl) chose for his dwelling a green hill before the feet of the White Mountains that were the south-wall of his land..."). Or, it may be his son Brego, as is stated in the Unfinished Tales.

Edoras? Freca and Wulf were killed there. But so was Haleth, the son of Helm, who was more royal than both Freca and Wulf. Maybe Helm's Deep? Helm and his other son Hama both died there.

I think he was one of those wingless worms, like Glaurung. I would say that is debatable at least - check out this thread: A Worm with Wings? Folcwine did indeed lose two sons in war; however, Theoden lost only (only!) one. That's enough for any parent, isn't it?
I believe Helm Hammerhand is the other half of the answer; his son Haleth fell "last of all, defending the doors" of Meduseld, and Hama died at Helm's Deep.
