The palantiri where made by the Master of the Stones, who lived in Tol Eressea. In LOTR, Gandalf says that it could have been Fëanor who made it.
They where at:
Orthanc, Minas Anor (later Minas Tirith), Minas Ithil (later Minas Morgul), Osgiliath (the Master Stone), Amon Sul, Annuminas and Emyn Beraid (Elendil's stone, guarded by the elves of Lûn)

Fantastic, burrahobbit. I expected this question to last a little bit more as I thought it was tricky, but the answer is perfect: near the Mount Doom, Sam looks at Frodo and Gollum and a wheel of fire on Frodo's breast warns Gollum about his destiny.
