I recall five times: Sauron's servants (Ringwraiths? can't remember), Aragorn, Frodo and Sam (twice, I think; Gollum tried to escape but they caught him again) and Faramir's soldiers.

P.S. Gollum tried to escape soon after Frodo and Sam caught him the first time; they were testing him by pretending they were sleeping and Gollum tried to flee (but didn't really get far).
