Well, I'm assuming that the Wormtongue bookmark has the little lizard thing on it. When Wormtongue considers leaving Saruman, Saruman reminds him and tells all the hobbits gathered around that Wormtongue killed Lotho, even though Grima denies it at first. It is at this time that he "realizes his wrong". He kills Saruman, and the grey mist that rises from Saruman's body "turns West then East then gone".

It is Thuringwethil. She was a messenger of Morgoth, and she was killed by Luthien and her bat-hame was used by the latter to get into Angband.

The obvious answer regarding the last part would be Gothmog, the Uruloki; I'm not entirely sure what you mean about kings though... Sauron killed two kings, but the rest of it doesn't fit him.

The correct answer is Gothmog you are right Gwaihir. The first line was supposed to refer to the original host of balrogs, that was reduced to just seven or three. The line about the kings was the two high kings of the Noldor that Gothmog killed, Feanor and Fingon. I'm sorry the clue wasn't very good.
