There were three, and possibly a fourth union of Man and Elf. Tuor and Idril, Beren and Luthien, Aragorn and Arwen, and one which occured in the house of Dol Amroth (Mithrellas, by name). Not all the Elves chose to be mortal.
As for the Halls of Mandos - the fea of the elf is summoned there
for a time, then re-embodied by the Valar in the restored former body.
