Is it Melkor

"Once seen never more"- refers to the battle with fingolfin, where he never again came out of Angband
"your name nor your form apear"- He was cursed by Feanor and the name "Melkor" was never used again, so it was cahnged to Morgoth
"Wise words slyly spoke."- deals with his deception of the Noldor in Valinor
