By sea from Pelargir to the mouth of Gwathlo, then up the river to Tharbad and then by land (following the road later called the Greenway) via Bree to Fornost?

Very good, Gordis! You are correct.
It was indeed this route. For reference, look into the Unfinished Tales, I can't look it up now, but in the passage about the port of Lond Daer, this route is said to be the most used and the fastest way possible from Gondor to Arnor; even from Minas Tirith, seemingly. It looks very interesting, and this is why I picked it, but it is said to be like that. Gordis, please give us a new question.
