There is a stream that runs from Ephel Duath to Anduin and at one point, it drops in a waterfall. There used to be another inlet of water that went underground and spilled out a cave that opened halfway down the cliff, but the Gondorians sealed this off, thus forcing all of the water to go over the falls on top. They had cut an entrance into the cave from the ground above the cliff. These stairs led into the hollow area that the original falls came out of. The only other exit was to dive out of the opening, thought the falls and into the basin far below.
